Your First Appointment

Please read the instructions carefully and fill out the forms completely to facilitate your visit to the CNI Neuromuscular Clinic. If you have any questions, please call our office at 303-788-1700.

Forms: Please bring all forms to the office at the time of your visit. Fill out the forms as much as possible before your appointment. Incomplete portions of the form may be filled out with the assistance our of staff.

Early Arrival: Please arrive at the clinic 15 minutes before your appointment to register.


Insurance:

    1. Neuromuscular evaluations are handled on an outpatient basis. Most insurance companies and Medicare cover all or part of the cost of the evaluation and treatment. Charges vary according to the extent of the evaluation.
    2. Insurance Card: Bring your insurance card to the appointment.
    3. Referral: If your insurance company requires a referral, bring a copy of the referral with you.
    4. Copayments: If copayments are part of your contract with your insurance company, payment is due at the time of the visit. We accept cash and checks, but are not prepared to accept credit cards. We ask that you come prepared to comply with your insurance contract.

Medical records: Medical records are important to your evaluation. Please obtain copies of pertinent medical records, laboratory studies, and x-ray reports. If MRI or CT scan studies have been obtained, please bring the films. They may be obtained at the scanning center.


Helpful Hints

  1. Write down a list of questions prior to the visit. We will do our best to answer all questions and concerns at the conclusion of the evaluation.
  2. Bring a relative or friend to the appointment.
